It depends on the antibiotic, but for most, it isn't true. Macrolide antibiotics such as erythromycin and clarithromycin may react with alcohol and affect the liver.
Alcohol has no effect on penicillin whatsover. The story that it does is an old wives' tale, possibly concocted when it was used to treat STIs (alcohol lowers inhibitions: if someone taking antibiotics for an STI were to have sex before the antibiotics could work, they would pass it on).

Its best to take most antibiotics on an empty stomach (1 hr before or 2 hrs after eating) with a full glass of water. Some antibiotics are hard on the stomach and the booze will compound the problem.
Some cold preps may have alcohol in them. Check the labels.
Dont drink if youre taking Flagyl (metronidazole)

good question. as i understand it, drinking alcohol will reduce the effectiveness of antibiotics. so, don't drink until you take that last pill! good luck.
