I can see if the anesthesia made her sick to her stomach and she is vomiting because of that but he blood in the stool does concer me a bit. Not to send her home with pain meds does too.
They should have given you a post op sheet as to what to expect will happen. I would definity call your vet, and quite frankly I would also give then a peice of my mind.
I would think that by now she should be feeling her self again. If there is an emergency vet you can get her too that is not a bad idea to do. She can become dehydrated so easily from all the vomiting.
Good luck!

The city that I live in has a 24hr emergency animal hospital.
I suggest that if your Vet is unavailable, than see if there is a facility near you that has 24hr emergency care for pets.
Also, 4 months is way too young for spaying. I have 3 dogs, and 2 of them are puppies. I had to wait until they were 6 months old before they could have this type of surgery. I hope your puppy will Okay!

Yes DON'T Wait! Your Vet sgould have an on call Vet. Contact them. If you can't find one there, find another VET that handles after-hours emergencies.
Get her seen, as this sounds like a life threatening condition, and she needs hospitalized and treated. Don't wait tol tomorrow or you may lose valuable time if she does not succumb to whatever illness, infection or condition is manifesting.
If you love your pets like I do, I don't spare any cost nor do I I allow anything left to chance. A good Vet will let you pay the bill off and work with you over time. Howevere; the money aspect is the least concern or should not be one if she means as much to you as most of our Dogs/Cats mean to our family of true Pet Lovers. She is part of you family and your life, you are her only link to life and health and happiness. Don't fail her now. She needs you ASAP!

They don't send them home w/ pain medication, they want the dog kept quiet, the pain will keep them slowed down.
The yellow bile means she is HUNGRY... Give her small bits and slowly get her eating again.
Diarrhea is likely due to the stress of the whole ordeal.
She is going to be lethargic, she just had surgery.

I'm concerned w/the blood in the stools. as for the others, they don't usually give pain meds for this & for the vomiting, only give small amounts of food for the first few days. your vet. should have told you this unstead of just exspecting you to know. give small amounts of food & watch the dog carfully. if the vomiting continus throw the night untill tomarow, call the vet. hope this helps some.
