http://hopstop.com is the best and easiest. You type in the location that you are starting from and the location of where you want to end up and it will give you the subway lines you need to use (and their location relative to where you are), a list of the stops you will pass, whatever transfers you need, plus walking directions after you get off the subway. It also provides maps to and from your stop and the place that you are coming from and going. And it also gives and estimate of how long your trip will take in time and miles.

MTA now has an excellent trip planner on their website. You can go straight to the trip planner at http://travel.mtanyct.info . You can enter either an address or a landmark/point of interest for the start and end of your trip, and you'll get detailed directions on how to get where you need to go.
You will still want to remember things like "uptown" and "Bronx-bound" mean "to the north" while "downtown" and "Brooklyn-bound" mean "to the south" and the fact that local trains stop on the outside tracks and the inside tracks are for expresses.

Then roll your mouse over the "Maps" bar up top, then down to "New York City Transit" and over to "Subway" If you really want to learn, you'll have to study it. I did, and it helps a lot. By the way, you can click on the stations, and choose a line, and it will tell you all of the stations along that line, and the cross streets where the stations are located.

It's easier that it looks...you have basically (2) choices - uptown or downtown...I would recommend at first riding the main trains

4,,6
Those should get you basically where you wanna go
