They're often interchangable in smaller companies, but larger companies may have different groups for each dicipline.
A programmer is the one who writes the actual code based on the designer's specifications. Again, the designer and the writer can be the same person.

computer programmer programs the computer whereas a software engineer designs and builds a software. taking an analogy programmer is just like a mason who puts the cement and labours for building. the software engineer is architect who designs building without ever concerning what cement will be used or what will be the number of labor involved, etc.
