Unless you are thinking of living on the Costa del Sol in Spain, I would opt for Italy. It's a lot more cosmopolitan, with more options and benefits for a foreigner.
Just keep in mind that, at some point, you will need to apply for a long term visa. Make sure your passport is not going to expire in the next couple years; leave the U.S. with a passport good for at least 3-4 years.
Italy is so diverse. It's like 20 different countries all in one. Even though I studied Spanish for ten years, I would rather live in Italy than Spain.
