Actually, if it's not lost or damaged in the course of duty and is the soldiers fault for losing or damaging it, we do charge them.

Would you like to be the officer that orders a platoon into enemy fire just to get back some of thier gear they dropped? I have a friend this actually happened to in Veitnam.
Today, if in combat or under reasonable circumstances, the military just takes the loss. No one actually the military would be efficient, it never is.

They do. If the gear is carelessly lost or damaged you are given a "statement of charges" and pay a portion for the items replacement. In war it is difficult to define careless as it occurs when someone is SHOOTING at you.
