Not sure about Europe, but almost certain the first in U.K. was a Hospital attached to the Priory of St. Mary's of Bethlehem, known as the Hospital of Bethlem or 'BEDLAM'
It was founded in1247 and remained there until 1676. The remains of the hospital lie under Liverpool Street Station, London
