For the mother, you will add a degree under the arm. It's easier to remember that as opposed to what is normal in what area.
If the childs temp is 99.4 or above under the arm, call the pediatrician. That is considered a fever.
When you call the doctor, tell then the exact reading on the thermometer AND tell them where you took it at...oral, rectal, axillary...
The doctor will calculate the actual temperature himself. The reason for this is that infants and toddlers running a fever could indicate a more serious issue than if it were, lets say, a 10 year old. Some doctors, specialists add 2 degrees under the arm(which is NOT normal)
You have less chance of confusion if you just tell them the actual temperature and where it was taken. this will ensure the doctor makes an informed decision in deciding how to treat your child,
If you have an doubt about fever, or anything, especially in children under 2....CALL THE DOCTOR!
It's just a phone call and it could make a major difference in your child's health.

You read the temperature as it is. It should record a degree lower than normal body temp. You don't need to subtract or add anything, the temp does that for you. Normal body temp is 98.6, normal rectal temp is 99.6 and normal axillary (underarm) temp is 97.6. Hope this helps, best of luck.
