You sort of answered your own question. When you hurt someone, even if they have hurt you it always comes back to haunt you. All you can do is learn not to hurt others in the future, to be more mature and wise. You are not in touch, so go on with your life and try to grow from this, don't let it cripple you. You know, most ppl have experienced this lesson, you are far from alone in having hurt someone you cared for. Sometimes when we are trying to separate from a loved one we hurt them to make the separation easier on ourselves, only we just make a mess of everything. It is an immature coping strategy.

It haunts you because you have not forgiven yourself for the sins you committed. You have to start there.
I recently had to track down a person after 20 years because I had to get it off my chest. It took me months. But, to forgive them and to ask for forgiveness and to also forgive myself for not being perfect was very cleansing.
This is what God offers us and what He expects from us. To truly love your neighbor, you must have a heart of forgiveness.

Because you feel like you failed and your ego is bruised. No, you did not fail and no your ex did not fail. Both of you had expectations of what you wanted from your life and they did not coincide. Is anyone to blame? No! Usually people try to put all the blame on their ex and exonerate themselves.

You know you were wrong. Perhaps write her a letter to apologize. Don't contact her, just write the letter, if you mail it or not is up to you.
