My fiance has a severe headache on the left side of his head and pain behind his left nasal passage. It really hurts to bend over and/or cough. I thought it might have been allergy/sinus but he said the medicine isnt working. Ive told him to go to the doctor since im not one but he refuses. Any idea's of what this could be and what should he do.

Get an ice pack, and have him put it on his face for as long as he can, then remove it, then repeat as often as he can. It does sound like sinus to me.*

Much pain is from muscles below is an example of what may help (based on headaches).
Begin with a couple swigs of molasses or a couple of bananas daily - magnesium (which regulates many things in the body) and potassium (a needed building block for muscles).
Drink at least 1/2 gallons of water per day. Running a body low on water is like running a car low on oil is the analogy the head of neurology at UCDavis told my husband about 10 years ago.

Now to the cause - muscles - your back, neck shoulders and head have tender spots. They are knots in the fibers of the muscles called trigger points. It makes the muscles tight which makes them press on nerves and other things causing the pain.

The cure - start with a professional massage, you will also want to go back over any place you can get to 6-12 times per session up to 6 times per day rubbing (or lightly scratching on your head) every where that is tender until the knots go away. The place where the skull connects to the spine press up under the edge of the skull (to get to those muscles).

It could be a migraine since the allergy/sinus medicine isn't working or it could be a symptom of something much worse. He should really see a doctor to get this checked out. There are several good medications used to treat migraines. Only a doctor can decide which treatment is best.

Since it's all centered in your head where the sinuses meet your airway, you might try a saline nasal RINSE (not a spray) A full warm water saline rinse goes in one nostril and out the other. It draws out all the gunk and shrinks the tissue (like swimming in the ocean). You can do this before, during, and after your infections as a symptom reliever and as a WASH to prevent future infections.

The buffered salt doesn't 'sting' as much as plain salt water. You can use a new method like Nasopure, or the vintage, Netipot

You can do this a often as you like with no bad side effects.

You don’t need a prescription for this therapy. So if you live in a state like mine, they’ve pretty much “outlawed” sudafed medicine. Drug-free is really the way to go on a frequent-use basis.

This might sound funny, but I read in a book that rubbing a lime on your forehead relieves headaches. Well my girlfriend and I were sick with the flu and all the medicines we were taking weren't working. So I suggested the lime, which you cut in half, and guess what, it worked. I also cut the other half, and sucked on it and it helped relieve my sore throat and settle my stomach. It will burn if you have a cut or pimple where the lime touches but its not bad. And you will notice that the pain will go towards one side if you don't get enough on that side.
So give it a try, you'll be suprised.

Sounds like a cluster headache. They are usually felt more behind the eye though. Usually there is a runny nose and eye watering with them. They are usually severe enough to cause men to want to go to the ER. But, if that is what it is, they last only up to 2 hrs.

If it is a plain old tension headache then he should take some advil and stay in a dark room.

It could be dehydration or High blood pressure maybe if you look up his symptoms on Web MD . He could see how serious it could be and that he should have a doctor check him out.

My mother ignored a headache and nearly died. She had a stroke and ended up having two brain surgeries to keep her alive.
Get him to the doctor soon!
