Nisei Lounge - Chicago, IL
This place is 2 blocks from Wrigley Field 3449 N. Sheffield to be exact. The bar is a hole in the wall in a serious drinking neighborhood with very large, very loud, very gimmicky (touristy)and very expensive places all around it. Yet, the Nisei holds out as a bastion of all that makes a dive a dive. Dark - check. Dingy - check. Dirty due to layers of carefully undisturbed filthy without actually being transferable when touched - check. Decent prices - check. Comradery - absolutely - frequenters of the Nisei know what it is and work dilligently to keep it that way. It is not quite a pirate bar (everyone growls "Rrrrrr" when someone new shows up) - they will except a newcomer, who has the right attitude - some stay for the day, others stay the rest of their lives.
They have a special drink that I have never heard anyone order - the only thing in the place that matches the Japanese name - the "Sake Bomb". They have a pool table complete with local hustlers. They show pay-per-view fights without a cover (or at least they did) when the neighbor is charging a $50 cover.
Someday this place will change - hope I'm not around to see it

Although I don't consider it a total dive bar, Kirby's Sports Bar in Fremont, CA is the best. They have 20 large screen TVs, 25 types of beer on tap, any type of hard alcohol you could want, pool tables, dart boards, outside enclosed smoking patio, very cute bartenders (Lisa, Annie, Melissa, Ana).

Ha-Ra in the Tenderloin in San Francisco. Owned once by a couple of boxers, you get a feeling of "old San Francisco". Carl the bartender is a San Francisco legend. Just don't ask for any frou-frou drinks. You'll hear it from Carl.
Cheap drinks, and cheap jukebox with great old tunes.

Liquor Lyles, Franklin and Hennepin, Minneapolis. No windows, red vinyl seats, pool table in the back, TVs, pull tabs, and very cheap. Free wings and herring at happy hour too!

Usually, the one I'm at. But my all time favorite is the Drift Inn in Bradenton Fl. The sign outside says it is the "friendliest place in town" so it must be true.
