There are three possibilities:
1) Yeah, they might think you're hot.
2) You might look familiar to them in some ways, like their friends, or someone they have met before etc.
3) When the person turn back, he/she must be thinking, "Man, I swear that fellow is so bloody fugly".

i think it means you have caught there attention but then again my friends and i would catch any ones attention when we walk down the street lol
