There are very, very few programs that are accelerated straight through medschool. Those programs are, I believe, generally 6 years. The vast majority of physicians will have a 4 year degree in which they can major in pretty much anything so long as they take:
1 yr. biology w/ lab
1 yr. general chemistry w/ lab
1 yr. organic chemistry w/ lab
1 yr calculus
1 year physics w/ lab
The different med schools vary on what prerequisite courses they require, but generally, those five classes will cover all your bases.

They go to college, which is usually pre-med or biology or some other sort of science. Then, they go to med school, then their internships. It's lots of schooling. But that's for medical doctors. There are many other sorts of doctors too.
Basically, "doctor" is an educational level, to which one aspires after a bachelors and possibly Masters degree. There are also post-doctoral forms of education as well.

In USA, Doctors have to do 4yrs undergraduate studies (bachelors) before getting into medical school (college), but in many other countries it is not so. In UK and India, they go directly from 12th grade to medical college for 4 yrs plus one year internship. In many european schools they also go directly from 12th grade, but the duration of medical college or school can vary from 5-8 yrs.

Ofcourse you do. I am a doctor. I received my bachelor's degree in biology. After that, I went to med school. After that, I did an intern. It took me about 10 years to become a doctor after high school which is average.

outside of the US they can be directly admitted to a medical degree track. I'm not certain of which countries. In the US it would not be possible. Although one does not need to achieve a bachelors degree to matriculate to medical school.

They definitely have to go to college (4yrs). Then, they must apply and get accepted into a med school. This can be very difficult to accomplish-- approximately 50% get accepted, I believe (or less!).
