You've hopefully gone through most of the testing that people will suggest. First off, I would suggest find an alternate doctor. Someone who is a specialist in gastro-enterology...a doctor who is specially trained in the digestive tract.
From what you describe, a blockage is what immediately comes to mind. If it has not shown up on an xray, you might require an MRI of the intestinal tract. Or, better, but more uncomfortable is a colonoscopy...where they send a camera up your hiney.
In some aspects, the human body is wonderfully adaptive....in others, very 'tight-lipped' when you need to know what is wrong.
Best of luck.
Now...going way far on a limb, if none of this comes back with results, you might try speaking with an allergist. There have been cases where people are allergic to cold, water and several other common day items. Who knows...but, considering the obscurity of your ailment, it couldn't hurt to try as a last resort.

I, also, would suggest changing doctors. You need to see a Gastroenterologist. (from the mouth to the rectum) They need to do an endoscopy--light down the mouth to the stomach-- and a colonoscopy--light up the rectum. If you have not had these, there is no way a doctor (or anyone) can tell you what is wrong. Some specialists only take referrals so you may need your doctor to refer you to a specialist or just try to get into one with a phone call.
Good luck to you.

I had a similar problem for 5 years or so. Turns out I was allergic to soy products. They didn't bother me at 1st, but over the years it got worse until it was a huge problem. I happened on the answer to my problem by chance.
Try eliminating foods and see what happens. I did a drastic change in diet when breast feeding my 1st because of allergies. Went down to potatoes and carrots - that's all. When his rash cleared up I gradually added things back into my diet to find his food problem. So, it can be done depending on how serious you are about finding a solution.
Hope you figure it out soon.

Find a different Doctor. You probably have some type of rare stomach disorder that the Dr doesn't see. When they hear hoofs they think horses, they need to be thinking Zebras.

Goodness. I am so sorry for you. I would get a new doctor and tell them you are not going away until they figure out what is wrong. Ask them what they would do if they were in your position. Good luck.
