Even though she's *69ing her calls, the phone company or police can get a record of the numbers that have called you. That doens't fall into real involvment by the police, it's something you're entitled to if you ask for it. Then you'll have something to show you're being harrassed and the police will have more to go on than your word that some crazy chick keeps calling you (right now you really don't have much to back that up with a phone full of restricted calls).
Forget HR, keep your direct supervisor updated every time an incident occurs at work. Tell your co-workers and ask them to observe how you're being hounded. Never let yourself be alone with her and stay away from her as much as possible.
As for hitting you in the back, you should have called the cops right then and there. You can try suing her for you phys. therapy bills with just your word against hers.

This will barely serve justice after what she's done to you, but better than nothing...
Anonymously e-mail her to no end! Create a new e-mail account with her name on yahoo or something and send it to her work e-mail address all day with random messages. You could include links about anything, all her faults; this may straighten out the problem a bit! Just try not to make it obvious that it is coming from you.
If not, it will drive her crazy having e-mails coming in to her from herself! Use the computers at your local library.

Unfortunately, you are probably going to be forced to have your phone number changed.
I would also go to the police, magistrates office, in person and request a restraining order and for harassment charges be taken out.
Lastly, if she hit you at work, you should file a workerman's compensation case. Witnesses are not required but you must have reported it to management.

Well, if they don't care that she's doing it to you, I say turn it around. Use star 69 and call this ****** repeatedly. Don't say anything, just call, wait for an answer, and hang up. Or, you could just turn your phone off at night and get a pager for family to keep in touch with you.

Start by changing your cell number or blocking hers. Try not to ever be alone with her. Make sure co-workers around you know what she is doing. Even if you just mention it to them, they will keep an eye on her even if it's subconsciously.

Turn your phone off. Let it go to voice mail, if she threatens you then get a restraining order. Get a new phone number and don't tell it to her. Kick her ***. What did you do to her?

File a complaint against the caller for unjust vexation so that the court will penalize her.

...it's not that the Police don't care... they don't have the ability to get in the middle of your problem... get your phone number changed....then, don't give it out to anyone... only those you absolutely trust.
