It's from a resturaunt in North Lake Tahoe, NV. Anyone know the recipe?

If you want the actual recipe, you're going to have to call Gar Woods [the restaurant I believe you are referring to, but I don't think they're going to give it to you] ...HOWEVER ... I have a similar, if not familar, option.
Ingredients
2 shots pineapple rum
2 shots banana rum
2 shots mango rum
1 shot vanilla rum
Method
Pour rum into a blender filled with ice and turn on the motor. Process until the mixture is slush. Divide among as many glasses as you wish.
Don't drink and drive.
