This was _just_ in the news. Hence the confusing answers here, I guess.

"On February 22, the U.S. Department of Homeland Security said that
children under 16 will not have to show a passport at the border. A certified
copy of their birth certificate and parental consent will be enough."

They've really cracked down on boarder crossings and passports. It seems too many terrorists were getting thru those boarders. No longer can people travel into or out of Canada without a passport. So my answer is Yes, a 3 month old baby born in Canada needs a passport to go to the USA for a short visit.

If you are flying he will. This is off the Passport Canada website:

Do children need their own passport to travel?
Yes. All children who travel (alone or with an adult) need their own passport. The International Civil Aviation Organization (ICAO) has recommended a policy requiring every individual who travels by air to have his or her own travel document. This policy has been put in place to combat the traffic of millions of children around the world who are often sold into slavery, child prostitution or worse. It increases the protection of children by ensuring that they have a valid travel document with a photo and other identifying information.

As well, for pictures:
What are the photo requirements for a baby?
A full front view of your child’s head and shoulders is required. Please note that the parent’s hands or arms may not be seen in the picture. Passport Canada is, however, lenient about the expression of a newborn (mouth open or smile

Parents traveling with children and infants are required to provide identification and proof of citizenship for each child. If both parents are not present, the traveling parent must provide a signed statement from the other parent allowing the child to travel out of the country. Failure to provide such evidence may make you unable to cross the border with your children.
