You should try setting up partnerships with hospitals in your area. You should also contact any local ENT clinics, get to know the doctors as they will prob. have a lot of referrals for you. Let them know that you are able to accomadate their patients. A lot of hospitals have a waiting period of at least a few weeks before they can get patients in, so stress to them that you have more prompt waiting times.

The easiest answer may not appeal to your morals. Find the best young doctor you can at another clinic or hospital that offers services similar to yours and hire them. They will likely bring 50 percent of their client base with them.
I also agree with previous answers regarding referals. It need not be solely from hospitals however. General practitioners, family clinics and pediatricians are likely candidates.
I do not recommend the use of incentives. The medical practice is not a field of services that can well suffer the stigma associated with incentive marketing and especially not the potential negative whiplash from travel incentives.
If you do go with a traditional advertising campaign I would probably target local publications that cater to the retired community. Hilight any benifits of your service that are of particular value to the elderly. Offer additional assistance with medicare and prescriptions etc.
Depending on your area, how mobile your testing equipment is and how much time you can spare, you may consider some event based marketing. Get in touch with local retirement communities and arrange to do free hearing tests/screenings on a specific date and time block. You will generate some word of mouth marketing with such an effort and it shows the general population that you are an organization that gives back to the community. The very concept of coming to them also will give some sense of values associated with doctors in the days where house calls were made.

You could offer your potential patients/customers an incentive to come to your clinic. An incentive that is popular with just about everyone is travel, almost everybody likes to travel. For example, you could say "Come to my audiological clinic and receive a certificate for a free hotel stay"

You need to create a sales and marketing system that fills your prospects at the top of the funnel and allows them to ascend to your high level support program. A continuity plan would also help you succeed and create a monthly residual income and referral program with incentives will mean you never need to actively advertise for clients again.
