Don't mess about with your cats ears at all....get em straight to the vets.

It sounds like a old hematoma. where the skin on the ear fills with blood. If its still puffy with blood a vet will drain it if not than its to late to do anything. That's what it looks like after it heals. it shouldn't hurt anymore. they get hematomas from scratching the ear (maybe ear mites or cat fight) a blood vessel pops causing this to happen. Dogs get it too.

Does he let you touch it? When you do, does he react as if it hurts? Is there any blood?

Has it always been this way, or did this appear suddenly? If it's the later, you'll have to get him checked out at the vet's - cats' ears are very sensitive.

I am sorry. I don't know how to help your cat. You should NOT touch
your cat's ears. If you do something with your cat yourself it may get worse. My advice is to go and take it to the veterinarian.

Take your cat to a vet. Does your cat go out doors, he may have been in a fight.

Possibly genetic. Some where down the line there might have been a Scottish Fold ancestor in his lineage. If he isn't upset when you touch it, it probably isn't anything to worry about.
