If pH is more than normal, the water is more alkaline.
If pH is lesses than normal, the water is more acidic.

So, the water can cause any mal-effects that an alkali or acid (in excess) can cause, normally, even worse as the alkali and acid are in their hydrated state which makes them even more potential toxins.

If you are drinking water directly from a river, pH is the least of your concerns. If you mean drinking that water after it has been treated, then you have no worry. Water treatment plants monitor pH. If there is some problem the plant can alter the pH or discharge the undesirable water.

I would worry more about other contaminates in the water.

Generally the pH will not naturally tend to not be very alkaline or acidic, and therefore not very harmful.

However water is a wonderful solvent and can easily pickup lots of harmful minerals and bacteria from the environment. I would worry more about heavy metals and that wonderful beaver dam full of bacteria upstream.
