fill it out, print it out, and send it along with a photocopy of YOUR photo ID.
Mine came pretty quickly.

You might not be able to get it online or mailed to you. You'll have to go in person to get it. Best suggestion, call the hospital where he was born and find out where you would need to go get it, and if they'll send it it you.
