There are so many ways I would have loved to have been asked instead of the boring old question. You should watch the 1st season of Laguna Beach...yeah, sounds stupid but they really had alot of original ideas in asking their date. I didn't see one person get turned down. I especially liked the one where the girl was at the salon getting her nails done and when she came out her car was being towed. It was obviously a prank to get her bent out of shape, but the tow driver gave her flowers and a card that asked her to the prom. As she was reading the card the guy comes out and they hug and she accepts. She was freaked out at first, but it was definately an original and funny way to do it. Who knows...good luck!

How about -
"Prom is coming up and I was wondering if anyone has asked you to go with them yet..."
If she looks creeped out, no harm done. You haven't asked her. If she responds well then you'll know where to take things from there.

This guy I knew from class had a bunch of helium filled balloons delivered to my door. Attached to the string was a note that just said - "pop them". So I did and in one was the word "will" in another was "you" and another "go", etc. (to prom with me). In some of them there was confetti. When I popped all the balloons I had to put the words together. After I figured it out, I got a couple of balloons and put my answer inside and had them delivered back to his house. It was really cute.

well i know how kids are these days, all trying to be cool and everything, but i still think a girl would be thrilled if you would approach her with a question like it would be my honor to escort you to the prom. All girls have a romantic side deep inside, good luck

the teddy bear w/ flowers idea sounds like a winner. hmm let me give you an example; when my sister got asked to prom the guy that asked her got one of those pool noodles [inside joke] and he wrote "will you go to prom with me?" on it and left it on our porch. i thought that was cute. maybe do something with her nickname or an inside joke.
