Yes i have heard of this, when i was in the Navy, i worked on helos, and i heard a horror story about some one falling in a 46 and getting impaled on the collective down in Florida. How true this is I'm not sure. It would be kinda like getting impaled by a baseball bat. That's roughly the thickness of the collective. Other then that i don't think it could happen unless you fell from a greater height.

I dont think you would be able to have the control stick go through you but if you are talking about the control surfaces meaning the rotor then I havent heard of it but I'm sure it has happened. Did hear a story about a ramp worker that chased a piece of paperwork for a flight into the propellor of a Saab. Bad ending...

Ha when i was in school to become a helicopter pilot i never heard of that but i guess it could happen.

Yes it has happened... A hard vertical landing or crash can cause the pilot or a passenger to be inpaled on the cyclic control... Happened more than once in Vietnam...

not exactly sure how you would do that in such a small cockpit, but no i haven't heard of that.
