You go to a particular post office (check out http://www.usps.gov) and apply for it (fill out the app in advance). You can pay an express charge to expedite the process (an additional $60). My wife and I did this before our honeymoon. We got our passports in one week. You're not going to get one by just going to an office.
Passport rules changed in 2006. It is harder to get one, and the newest ones are biometric. Do it now rather than wait last minute.

You can submit an application at any US post office - they will tell you to allow 6 weeks. It may come in 4 or 5 weeks, but there is really no way around the system (that I know of).
A cruise line will tell you 6 weeks. I'd be very suspicious of an office in Miami getting passports out any faster than 4-6 weeks.

Selected Post Offices have passport service. You can pick up the form there, and some even have photo capability. (So does AAA.) If you have proof of your upcoming travel plans and are willing to pay an expedite fee, you can get very fast turn-around. Even routine is not bad - I just renewed mine, and had my new passport 2 weeks after I mailed in the renewal application. Getting a new passport rather than a renewal takes longer. If you were here in DC, there's even a State Department office that you can go to for same day service.
