Help me find a chocolate company that can make and wrap my chocolate bar?

I want to know how to make chocolate bars but from a Candy company that has the equipment to have make small quantities of my chocolate bar. I also would appreciate any information about how to ship chocolate. Distributors, packaging and best options.
