Please explain this to your dentist and if they are good they should refer you to the hospital, sometimes they can do fillings only it may take some time as they will let you rest but that would depend on which teeth need fillings. I needed a small filling only my dentist had a quick look and wouldn't do it. DO get the TMJ sorted out as mine wasn't for a while and by having work done by a not so good dentist it caused trauma. I do know about these things and no am not qualified but I suffer from this and am a very bad case, awaiting a jaw replacement on right side and not many are done in the UK, if you check my questions you will see and believe me as a sufferer I would say I am qualified regarding the pain as just that a sufferer. I have TMJ and I was suffering from daily headaches and severe migraines requiring hospitalization. I also had a lot of pain in my jaw area and can not open my mouth very wide. My dentist diagnosed me with TMJ and made me this device called an NTI because I have a bad habit of grinding my teeth during sleep. Once I got this amazing little device, I quit having daily headaches and only have the ocassional migraine. It is still painful to open wide and for long periods but my dentist will work a few minutes and then give me a few minutes to rest and then we start the whole process over again.A lot of my jaw pain has disapperaed since I got the NTI. Also stick to soft foods and don't chew gum or eat hard candies and other hard stuff.
There is also sedation dentistry being used now--both oral and IV so that is an option that you can look into.

TMJ can be very very painful and the symptoms of tmj are
ear ache,tender jaw muscles, temple and cheek pains, popping/clicking in jaws, locking of the jaws,frequent headaches and as you've pointed out, difficulty in keeping the mouth open.
Explain the symptoms to your dentist and then he/she will be able to try and resolve the problem.
My dentist normally refers his patients to the Maxilla Facial departmemt at the local hospital.
As for the fillings, why not opt for sedation if you cany keep your mouth open for long periods of time.
Take pain killers for the discomfort and try and rest your jaws as much as possible, avoid eating gum also and stick to soft foods.

Since dentists normally treat TMJ, call and speak to the dentist's office about this problem. Go in and have the TMJ evaluated and treated, then the dentist can treat your cavities.

Have your dentist prescribe Valium (diazepam) 10 mg and take one about 1-2 hours prior to your dental appointments. Valium is a good muscle relaxant. Ask your dentist to work in short intervals of only a few minutes and then give you a moment or two to close your mouth. If 10 mg doesn't seem to be sufficient, then try 12.5 or 15 mg dosages.
