I can't seem to stay focused on one thing. First I'll be doing something and then my mind begins to wonder .And then I'll start another project and so on and so forth . I know I probably have Adult ADD but is there anything besides medicine that I could take or some kind of exercise ( mental of course ) that I can practice?

Practise meditation. A breathing meditation is easy to learn and will train you to maintain focus. Close your eyes, focus on the feel of your breath coming in and going out. When your mind wanders just refocus on your breath. Do this 15 minutes a day to start and build up to 30-45 minutes.
