Start by organising yourself. Set weekly goals and try to achieve them, don't worry about long term if you go week to week the long term will fall into place.
make sure to eat healhy every day, start with breakfast, try not to skip it. If you can exersize in the mornings it's best, even going for a walk around the block is a great start.
For school try to do your homework the day you get it or the next day, never wait until the night before it's due, and ALWAYS do your homework, even if it's boring, or your not being graded on it.
The best modivation is when your hard work starts paying off. Every time you notice yourself being lazy go for a walk around the block, if you didn't have time then you wouldn't be lazing around, it'll give you a boost of energy to start on homework or studying, and it's good for you.

Set a goal for yourself; a goal that can be a real reinforcement of your desired weight lose. For example, that goal my to get healthy or physically fit enough to go on a long backpacking hike, or to fit into a new dress or whatever it is. With a motivating goal, it can help provide the necessary incentive.
Then, every day, remind yourself of the progress you are making (or not making) toward the goal. Evaluate your progress regularly.
When you have a slip up, do not beat yourself up and feel all is lost. Just get back up in the saddle and keep going. If you were on a long journey and took a wrong turn off, you would not go all the way back to the start to begin your journey again. No, you would just get back to the right road and start again. Likewise, if you have a slip up, don't feel all is lost and think you need to start over. Just get back to where you were and keep going.

Next time you look in the mirror ask the person you see if she would like to be fat and stupid or smart and healthy. Then ask her if an hour of exercise and an hour of studying a day are better or worse than 24 hoursof feeling bad about herself a day. You should have your motivation. It takes so little to improve yourself and it costs so much to do nothing. Good luck! I know it seems hard but you can do it.

My suggestion that got me motivated was a website called go.heart.org this site tells you if your at risk for certain heart problems and depending on your weight and age tells you other things you may be at risk of!! Check it out its very helpful!!
