Depends what drug you've been using, how long, and how often! You will be okay if you abused anything but marijuana. Marijuana stays in your system the longest and if you've been a daily user for over a year you might want to let them know ahead of time because I have known some people who have been known to fail after giving it up nearly 6 months! Of course, they go beyond the usual pothead!

I would say if you have been truly drug free for four months, then the answer is no. To my knowledge the drug that stays in the system the longest is marijuana, that is due to the fact that it resides in your fat cells. The higher your body fat the longer it stays, but even with a very high body fat and very low metabolism the longest it stays in your system is around 90 days.

Only worry if the test is a hair sample. Urine won't show any drug use.
