I think it's when you're not concetrating on what your doing eg. eating and taking at the same time, but here's some other ideas -

Generally keeping our tongue away from our teeth is an automatic thing, but sometimes our brain loses concentration or when we are tired our tongue may move slightly slower than expected and we bite our tongue - and yes, it hurts!

when your eating or chewing to fast, then you bite yoyr tounge accidently
