The following is my suggestion:-
1) Keep reading quality newspapers and magazines in English regularly.
2) Keep watching the English News channels regularly and follow the way the speakers speak English;
3) Become a member of the British Council Library. They have even on line service. Join their course on English language.
4) Try and speak English with others when occasion demands and do not hesitate for fear of faltering. A child learns to walk after encountering many a fall.

There's no shortcuts to vocabulary. You must read as much as you can. I advice reading books in subjects of your interest. I find that when I try to read those "good books" that everyone suggest, I eventually give up. Reading allows you to learn vocabulary in context.
I have a friend from Mainland China who memorized the Oxford dictionary. However, most of the people in school couldn't understand him because of his strange sentence structures. He's actually a science genius, but he couldn't express his ideas. He couldn't even handle basic conversation with teachers. Like he would miss his exams because he doesn't know what "Friday after the next" means.
To improve pronunciation, you can go to voice classes. Most of my musician friends can pick up language fast because they are good at listening. Listen carefully how each letter is pronounced. Practise. Practise. Practise.
Conversation classes offered by the British Council helped me a lot on my pronunciation. http://www.britishcouncil.org/
Also try the BBC languages website.

My last and most important tip is that you must have very THICK SKIN. You can't learn unless you've made mistakes.

Try to understand and in case of even little bit difficulty, see meaning in dictionary.Example: See the hoardings of advertisement, try to read the writings on rappers of daily needed items and watch these writings again on next purchase. Today I was shaving observed new word on Shave Cream Wrapper " Nick" mean small cut. Now meaning of Nick will not erase from my memory & it will got further force when I will see this wrapper next time.

A good way to improve is to listen to recorded stories, available from the library. This will improve both your vocabulary and your pronounciation.
Consider writing to someone on a regular basis to improve your written skills. you could do this by email or regular mail. Ask them to point out where you are making big mistakes in your writing.
Good luck. Your English seems pretty good to me! Stick at it.

It would be good if you could get a teacher to teach you spoken English.
Meanwhile, you can listen to news in English on the radio or TV.
Take a dictionary and select 5 words at random and learn their meanings and see how they are pronounced.
Then use those words in sentences.
Read the news paper aloud .
Join a library and read books.
