It looks like I need to remove some fascia inside the driver's door to expose screw-heads?

Correct.

Use a flat-headed screwdriver to carefully prise off the small triangular trim piece on the inside of the door - where the mirror is mounted on the outside. Behind this you'll find the electrical connector (if the mirror has electric adjust and/or heating) and the mounting bolts - most cars have three bolts.

prise of interior mirror trim using a small blade screwdriver,revealing mirror retaining bolt or bolts this is usually a torx head.you may have to remove the interior door panel to get to electrical connection

yes, and if you break the triangle while pulling it out.. just glue it back ;-)
