For me, once, it was before I opened my mouth...and the answer was so overwhelmingly quick, prompt and apt, that I think he was waiting a long long time for the question.

There is a verse in the Bible that does say that He answers our prayer while we are still asking. I can't remember where it is, I think Psalms.

I have heard that studies were done < I wish I could remember where> praying in the 1990's for people who had surgeries in the 1970's and it was supposed to have worked improving the outcomes. I do not claim to understand how it all worked, setting up the studies and all, but it sounded intriguing.

It depends...you can receive the answer as you said, later or even...never if what we are asking for it is really no good for our lives.
The prayer gives us power if we thank God, if we praise Him...He knows our needs and He takes care of us. All we have to do is to listen to HIS WORD and live a proper life!

Prayers are answered by God in His time. Sometimes quick, sometimes it takes years. But there will always be an answer, either "yes" or "no".

my Gods answer my prayers... Most usually I receive either some sort of a help or a sign.

if a prayer is answered before it is spoken, is it still a granted prayer request? Or mere coincidence?

It depends on how strong (feelings) and clear (picture in your mind) your desire is. It also depend if you are canceling that wish with modifications, conditions how it should appear or doubts.
