Means that they pay a high price & it doesn't always necessarily mean money. It could be something else of high price, such as a expensive race horse, or art - painting, or even a human. As for the therapy part it is a lesson to be learned

Therapy is like counseling. It's usually with a psychologist or phsychiatrist.
There's also physical therapy, which a person goes through after surgery or an injury, but this type is usually covered by insurance.

Paying through the nose is when you think you pay too much and you are working hard to pay for somethng.
If you really need soemthng, and you cant afford it,and you make extra effort for paying for it, you are paying thru your nose.
The above sentence means that Americans pay a lot for therapy, sometimes which they cannot afford.

I am going through therapy for a broken wrist, broke off the hand completely. I am paying through my nose, It cost $180.00 per therapy treatment.
