The widget is filled with liquid nitrogen before the bottle or can is sealed and then once it evaporates it creates pressure in the bottle. Over time the pressure actually forces beer into the widget through a small hole in the side. Once the beer is opened the instant change in pressure causes the beer inside the widget to spray out inside the bottle and causes the beer to foam up and create a better cloudy foam and head that you see in the commercials. Since Guiness is so thick it has less dissolved CO2 suspended in the solution than most lagers so it needs a little bit more agitation (provided by the widget) to take full advantage of the CO2 that is there. You could get the same effect almost by simpley shaking the bottle after you open it.

the plastic things are there to make the bottle pour like it comes out of a keg. those are the draught bottles.
