I would check for the incision, and if there is an incision and you still see 2 balls, I would wait a few days and see if it is swelling if not check with the vet.
If it is as the other girl explained an erection than possibly he did not get completely nuetured a nuetured dog should not get erections. I just learned that when my male passed away from prostate cancer. He was supposed to have been nuetured but he obviously wasn't fully nuetured because he would get erections and the obvious he got prostate cancer. That is impossible for a completely nuetured dog. That is at least what the vet told me.

No, the testicles are actually not there..they have been bumped off. What you will discover is swelling from the surgical technique. Week via week you will see that the swelling will circulate down. Neutering/castration skill to eliminate the testicles. The testicles have been bumped off and thrown away. the only difficulty left in the back of is the scrotum/sack.

If he was truly neutered (do you see an incsion?).... then it is typically one of two things:
1. a lot of people see the scrotum or ball sac filled up with post operative swelling (and it looks like he was never neutered)
OR
2. 2 ball like structures will form on either side of the penis when they have an erection--- making it look exactly like 2 balls.
i get calls like this a lot.
but the key is-- is there a post operative incsion-- confirming the surgery was done. ?

was he neutered or was he surgically sterilized? if it was later he would still have his balls but not able to impregnated a female dog
