Depends on the school - at my daughter's school you need 6 GCSEs minimum, grades A-C. In the subjects you want to study for A level you need at least a B, and if you want to start a new subject at A level you have to have an average of a B grade across the board in your GCSEs.

Yes, it will depend on the school, and even the subject - ask your teachers! However, generally speaking, you will need a C or above at GCSE to take a subject to A level, and most teachers would expect you to have an A or B.

When I did mine at 16, back in 1995, you needed 5 GCSE passes at grades A-C to go onto A-Levels. I'm not sure if it's changed though.
