And what exactly does meth do to the brain?

Not all meth abusers have delusions of bugs on the skin. As a former meth abuser (with over one year clean time) I would rub my hands over my skin and pick at the hairs on my skin. I never saw bugs on my skin. No sleep and no food going into the body can cause these delusions though. Meth releases dopamine (which is something you naturally produce) causing a sort of dopamine overload... which makes the user feel very good. Soon after though, if the use of meth is stopped, your brain has a hard time producing dopamine on its own without the meth. This causes some depression/anxiety issues. (Dopamine is also released when you are head over heals in love... which is why when a relationship ends you are down in the dumps). I know that because of my meth use I now have some anxiety problems, depression issues, concentration problems and much more. Meth fries your body from the inside out.
