If a toothbrush/paste is absent the best thing to remove plaque is a wash cloth or even your finger to scrath the plaque off. All mouth wash does is kills germs that cause plaque and freshens your breath but plaque itself has to physically be removed. A good thing to do is wrap your finger in guaze, washcloth or a t-shirt and rub your teeth with it. Then rinse with mouthwash if available.

If you absolutely have no access to a toothbrush and toothpaste then chewing gum with xylitol may help stop the adhesion of bacteria to the teeth. Mind you if you can't get a toothbrush then you probably can't get gum either. If the reason for not brushing is that it is uncomfortable start by using the softest and smallest brush you can find - even a child's brush is better than nothing and very gently brush/massage you teeth and gums.
