Do i have to use two forms one for federal and one for state???

The 1040EZ is for Federal returns only. Each state has their own forms. Some may have one called "1040EZ" but it's still not the same as the Federal form.

The 1040ez form is for Federal filing purposes only.

You would file a different form for your state.

Yes, a state form is always separate from federal, and they are sent to different places. Filing for federal doesn't take care of a state filing, and vice versa.

no, just one unless you are claiming earned income credit. Dont forget to claim $30single or $40joint for telephone excise tax credit. It requires no supporting documents and everyone can have it.
