i have this really bad. Caused by grinding my teeth in my sleep which is a common problem - for me caused by stress. i had some physiotherapy and a plate made so i can't grind my teeth in my sleep and now it's more or less gone away, but this is one of the most common causes.

usually the spongy cushion that is call the TMJ is out of place or too small. The joint doesn't have the cushioning to protect it
