it can be, but not always.
lets say theres a little girl thats trying to help a baby bird thats fallen out of a tree or something.... someone might say "well, bless your heart! what a nice thing to do".
OR, you might talk to an older republican person and say "i think the world would be a lot better off without the war in Iraq" and they might say "bless your heart child, i know you mean well, but you cant understand whats really happening over there".
in the first instance its just a nice thing to say. in the second one, its condescending because its like saying "i'm dismissing your opinion and making it look like i'm taking pity on you by saying something nice first."
so, it all depends on the delivery of the phrase.

I had an acquaintance once every time something was not going well with me, she would say, "Oh Bless Your Heart." I knew she didn't care about anyone in this world other than herself. One time things were going so bad for me and i had had a few margaritas, She said, "Bless Your Heart" and I told her where she could shove it. Needless to say I've not seen her since. i hate the term. But that's just me.

I find "Bless your heart" to be the warmest and nicest thing anyone could say to me.
I absolutely LOVE the expression. Glad I live in an area of the south that I get to hear it often. It never fails to make my day nicer!
Around here it replaces Thank you, I'm sorry to hear that, or it's a good thing you are doing.

It can mean "Thank you" or "that was nice". Or it can be a way of being nice or whatever to someone that you see as just having commited some sort of faux paux. That's my understanding of it. But I really don't hear it much.

I'm atheist and I find it ridiculous. Not insulting though. But it makes me chuckle. I find it akin to a 12 year old saying "I hope the tooth fairy brings me lots of money". It's cute but meaningless.

It all depends on the tone of voice. Some people actually do say that in earnest but some do say it to irritate you.
