No you dont need to use cooked chicken. Simmer the chicken in chicken stock, until it is cooked, pull it out, add your vegetables and flavors, chop up your chicken or pull it apart, then re-add it to the pot. This will give the chicken stock a richer flavor.
Ive never made chicken noodle soup, but Ive made Pot Au Feu, which is a chicken soup with vegetables. And this is how I make it. I also add an onion pique to it, an onion sliced in half with the paper on it, with a bay leaf stuck to the onion using a whole clove. Its a delicious soup too.

When I make it I use a whole roasting chicken (uncooked), covered in water. I add all my spices (lots of garlic), vegetables and bring it to a boil and simmer it for about 4 hours. Then I strain it, pick the meat off the bones (set it aside) & let the stock cool so I can strain off the fat.
Then just before dinner I will reheat the stock, add the meat & any veggies I want & the pasta goes in for the number of minutes it needs to cook. A squirt of lemon juice adds some nice flavour to it as well.
You don't need to precook the chicken if you have the time to let it cook in the stock slowly. You just have to make sure the chicken is good and clean before you add it.

Yes.
Usually when I make soup it's from leftover chicken or turkey, so it's already been cooked. Boil the leftover carcass in a gallon of water, let it cook & after it cools pick off all the meat you can. Then return the meat to the water you boiled it in, add some chicken gravy and/or chicken stock, carrots, celery, and whatever else you might like. Then add the noodles and let them cook until done.

Yes, I usually use leftover bits of chicken or turkey. I cut up fresh onions and celery and put that in there. Sometimes I throw in carrots, either fresh or canned. Sometimes I throw in a can of mixed vegetables, too. I've put rice in there but be careful because rice absorbs water very quickly and you may have to add more water. Potatoes or noodles are good in there too. I use a teaspoon of this mix of spices for each cup of water in my soup:
2 tablespoons parmesan cheese
1 tablespoon parsley, dried
1 teaspoon paprika
1 teaspoon onion powder
1/2 teaspoon oregano
1/2 teaspoon garlic salt
1/2 teaspoon sweet basil
1/2 teaspoon salt
1/4 teaspoon pepper
(I store this in the refrigerator. I also use these spices to make boneless chicken breasts and they'd probably go good on pork chops too)

You do NOT have to cook the chicken first to make soup.
However cooking the chicken first, usually by roasting, or sometimes braising (in the soup pot) makes both the chicken and the soup taste better.

No. If you put raw chicken into a pot with water and bring it to a boil then turn it down to a simmer for 20 to 30 minutes. Add in onion, garlic, and other veggies for a hearty stock. Add in a fare amount of salt also.

Not if you buy a roasted chicken. You can just take the meat off the bones and add to your chicken broth that your veggies have cooked in.
