The meaning is similar but not the same. The first one, "Complete the sentences with an appropriate form of the words" implies they're only requiring ONE appropriate form of the words. The second one "Complete the sentences with appropriate forms of the words" indicates they're asking for SEVERAL forms of the words.

Pretty much--however, the first "AN appropriate form" seems to limit you to a single form while the second allows you to use more than one form. If you have time before the assignment is due, ask your teacher for an explanation.

They are not the same. "with AN appropriate use of the words" means there is only one correct answer. "with appropriate forms of the words" means more than one answer is correct and the asker wants all possible correct answers.

The second one is correct, though I suppose both imply the same meaning. However,"form" shouldn't be singular if you are implying multiple forms of multiple words.
