Normally that's the way it is in American pro wrestling. You go to Japan, that don't even have things like that over there. And of course there are a few exceptions here in the States. And if you go to Canada, every Canadian is face and every foreigner who wrestles against a Canadian is a heel.

The problem with faces and heels is that if the crowed decides they hate a face or love a heel, there is little the promoter can do about it. A few years back, everybody loved Kurt Angle, even when he was wrestling against John Cena. They did everything the could to make Kurt look like an asshole, just to stick with the whole face/heel thing.

No! Not in WWE, anyway. I'm not sure about any of the other brands of wrestling, but in WWE, they often have face vs. face matches. It draws more attraction. Like the No Way Out match...Batista, Cena, Underaker, and HBK were all faces (to a certain degree, Batista and Cena were certainly not the favorites, but they were faces, nonetheless) when they walked in the building for No Way Out. Batista DID kind of turn heel, but he's not full heel yet; right now I'd say he's getting kind of played back and forth...hope that helps!

Yes, most of the time. If there ends up being a face vs. face, one will eventually turn themselves heel, same for heel vs. heel. And it depends on what fans you talk to too, because in those kind of scenarios, they might disagree on who's the face and heel.

No. There is also "Tweeners" who are between Face/Heel status. The most famous one would be "Stone Cold" Steve Austin who was a hated heel going into his match with Bret Hart @ Wrestlemania 13 (1997). Because he didn't submit to Bret Hart in the I Quit match, fans were cheering him as if he were a Face & basically forced the WWF to push him as both.

No for example at wrestlemania HBK vs John cena will be a face vs face match. Also Undertaker vs Batista will be a face vs face match.
