*No jewelry is fake, but I mean fake in terms of fake gems, like cubic zirconas, etc.

Assuming the giver knew me well enough to purchase something I would love, I would keep it, and love it. If the giver did not know me, and I didn't like it, I would appreciate the thought behind it and perhaps pass it along to someone whose taste for which it was a better match.
The bottom line is to be appreciative of the gift, not to take an attitude of arrogance or vanity that I'm only worth genuine gems. Heartfelt is heartfelt. Not everyone can even afford to buy gemstones.

That depends - did they tell me it was a genuine diamond? Or are they up front about it? If up front, then I'll enjoy it for what it is and wear it. One of my favorite rings is cz - it was purchased by my husband two days before he was hospitalized for anxiety attacks caused by large crowds. He went to the mall in the midst of Christmas rush to buy me the ring so I'd have a Christmas present, even though he knew he was going to be sick. That's love, and I don't care if the ring was PLASTIC.

i would definitely be appreciative, because i'm a very firm believer in "it's the thought that counts"

i would wear it all the time too! unless i was allergic to the metal or something, which would be very unfortunate.

i think its the thought that counts. if theres good intentions behind it then you cant be pissed, you'd come off as superficial.
