It's a reality show, but if you watch carefully, you'll notice clever editing by the producers. If you hear someone's voice but the camera is focused on the other person's reaction rather than the speaker's mouth, it's often not the reaction just as that person is speaking.

I think a lot of their activities are somewhat set up as well (like when multiple cast members go out to dinner together), but then their interactions from there on are unscripted (possibly with some prompting as to topics to discuss).

There was a great VH1 special going behind the scenes of reality shows. Watching that made me notice how simultaneously real and unreal reality shows are.

It's all real but I'm sure the producers make them over-exaggerate some of the scenes. It's a reality show and you can't expect much from it.
