There's this emergency contact form I have to fill out but I don't know my hospital information. I don't know where my birth certificate is. Can someone please help me? Thanks

All that information will be on your birth certificate. Contact your local health department or court house to get a copy, if your parents cannot give you that information.

Well, you could ask your parents or contact them if u dont live with them, they would know where you were born, and if they don't know who your doctor was, you could probably call that hospital for your records.

Go to the social security office and they can trace it through your SS number.
