Well, any organism that causes a disease is a pathogen. Plasmodium vivax causes malaria, and it is transmitted by mosquitos. So, I'm guessing all of these are correct.

I believe the thing that carries the pathogen is called the 'vector.' So fleas and rats, for example, are vectors for the bubonic plague.

Anyway, the very thing itself that causes the disease is of course the plasmodium pathogen. The pathogen could still theoretically be transmitted by vectors other than mosquitos (I'm sure it is transmitted by other types of blood-carrying insects). The mosquito is really not cause in itself. It could only be so if _everyone_ who got bit by a mosquito got malaria, which of course they don't because many mosquitos do not carry plasmodium.
