what causes a 95 yz 125 to fall on its face when the power band kicks in

Faulty power valve.
Remove the front and right covers on the cylinder. Operate the power valves by hand. They might be so dirty with carbon, that they won't move.
If they operate freely, then I would check to make sure the governor hasn't exploded. You'll have to remove the right crankcase cover to check it (clutch cover).
